About New Mexicare Caregiver Health Model (CHM)

The New MexiCare Caregiver Health Model is a program to provide financial assistance and training to caregivers who are assisting friends or family members with daily activities due to physical or cognitive limitations. The goal of the program is to allow older adults to thrive in their homes and communities and reduce nursing home placements and emergency department visits.

The program provides up to $1,000 per month to assist with the payment of caregivers who provide an array of services and supports that assist with activities of daily living. Participants must require assistance with two or more activities of daily living such as bathing, feeding, dressing, toileting or transferring.

Funding may be utilized to reimburse caregivers or providers who provide one of the following services:

  • Personal Care
  • Goods & Services:
    • In home or agency-based Respite
    • Transportation
    • Home Health & Safety Monitoring
    • Chore Services
    • Prescription co-pays or deductibles

This program is a self-directed model of service delivery that allows the participant:

  • The freedom to choose the services and supports they need to live independently, in their own home and setting of choice.
  • The flexibility to develop an individualized service plan and schedule of care that meets their needs.
  • The ability to hire caregivers that they trust and who have successfully completed a criminal background check.
