As these programs continue to grow and new opportunities develop in other states, ARIS Solutions continues its involvement with VDC programs to provide Veterans and their families the Financial Management Services they need to continue care in their home. Taking the burden of tax filings and payroll processes off the Veteran allows them the time to self-direct individual care needs and provides the opportunity to focus on what matters the most to them.

The Southwestern Connecticut Agency on Aging (SWCAA) is the trusted source of advocacy, information, and access to care for older and vulnerable adults that provides resources to strengthen the regional aging network. The Agency supports family caregivers by providing temporary care and assistance for eligible persons designed to give caregivers a much needed rest. SWCAA is also an “Access Agency”, under contract with the State of Connecticut, Department of Social Services, to provide care management to Medicaid clients who prefer to receive home and community-based services rather than institutional care. SWCAA’s care managers are social service professionals who help assess and build a person-centered plan of care designed to help elders, persons with disabilities and persons with acquired brain injury.
